One way many caring people choose to honor the memory of a lost friend or family member is through an unrestricted gift to Banner Health through Banner Health Foundation or Banner Alzheimer’s Foundation. Gifts in support of Alzheimer’s research and prevention should be directed to Banner Alzheimer’s Foundation.

When families choose to make Banner Health the recipient of gifts in memory of a loved one, we personally express our gratitude for their foresight, vision, and kindness. And when donors make memorial gifts in the name of that loved one, Banner Health's Foundations respond with a note of thanks and acknowledgment. Additionally, a designated family member will receive a list of donors with their contact information, so the family can acknowledge the gifts. 

To make a memorial gift in honor of someone, you can donate online using a credit card.

If you prefer, you may send a check made out to either Foundation, noting the name of the person to be honored in the memo line, to:

Remember to indicate the name and address of the family who should be notified.

Suggested Wording for Memorial Giving

You may wish to indicate in a loved one's obituary that you would like gifts in their memory to be donated to one of Banner Health's Foundations. Here are suggestions for expressing your wishes: 

  • In lieu of flowers, the family requests that gifts be made in honor of _______________ to Banner Health Foundation, 2901 N. Central Avenue, Suite 160, Phoenix, AZ 85012.
  • In lieu of flowers, the family requests that gifts be made in honor of _______________ to Banner Alzheimer’s Foundation, 2901 N. Central Avenue, Suite 160, Phoenix, AZ 85012.
